Your DPC membership includes unlimited office visits during regular hours, unlimited telephone consultations, unlimited email communication, and unlimited telemedicine visits, all at no additional charge. No copays, no surprise bills, no checking whether something is “covered.” You simply reach out when you need us.
Full Spectrum Family Medicine
We care for the whole family, from newborn well-child checks and sports physicals to chronic disease management for conditions like diabetes, hypertension, asthma, depression, and many more. Need to be seen for something urgent? We offer same-day appointments for acute issues, and we’re also available nights and weekends.
In-Office Procedures, Included
Many procedures that would require a separate specialist visit (and a separate bill) elsewhere are included in your DPC membership. These range from skin biopsies and cryotherapy to joint injections, EKGs, wound care, splinting, and even acute migraine treatment. In total, over 20 services are covered as part of your membership.
On-Site Pharmacy, Labs, and Imaging at Reduced Cost
We dispense over 100 medications right in our office at discounted wholesale prices, typically around one-tenth of retail pharmacy pricing. Point-of-care lab tests like strep, flu, urinalysis, glucose, and thyroid panels are done in-office and average just $5-10 per test. And when you need imaging, we offer discounted X-rays starting as low as $40, along with mammograms and bone density testing.
Affordable, Transparent DPC Membership Pricing
Monthly membership fees are based on age. Adult memberships range from $75 to $110 per month depending on age. Children are $55 per month, with a discount to $44 per month when two or more kids enroll with their parents. There’s a one-time enrollment fee of $99 per patient or $222 for a family. That’s it: no hidden fees, no deductibles, no confusing EOBs.

We have clinics in Middleton, Madison, Fitchburg, and Janesville, with hours Monday through Friday from 8:00am to 5:00pm and evening, weekend, and home visit availability by appointment.
